 Our practice uses TherapyAppointment 2.0.  Patients have access to a portal to see account information.

  • Receipts and superbills  can be accessed in REAL TIME on the portal.  

  • Text reminders will come to you 3 days prior to the appointment. 48 hour notice for cancellation is requested to avoid missed appointment fees.

  • You can send emails and attachments directly through the portal.

  •  Patient intake forms (links below) are still required for all new and returning patients.

 

Please read office policies carefully. 

Our staff is committed to your mental health care.  In order to continue to fulfill our commitment of care, patients must also complete the financial commitments for services rendered and appointments which are scheduled.  A credit card is requested to be retained on patient chart to charge for patient responsibility amounts and penalty fees.  Penalties for late cancellations and missed appointments will be enforced due to the increase in demand for mental health services. Thank you for your understanding and consideration of the providers and other clients seeking help.

  • Appropriate Intake forms (below)

  • Report cards, K - current grade level or home school sample work

  • Copies of previous evaluations, testing, FIE (shows tests and scores), 504, IEP and ARD plans (if applicable)

  • Insurance identification card

  • Form of payment


Providing the above records is necessary in order to provide the best clinical assessment.
