Consumer Access to Health Care Records and Complaint
Information

Texas law requires licensed behavioral health professionals to make certain consumer
information easily available. In compliance with House Bill 4224 and Texas Health and Safety
Code §181.105
, the following information explains how clients may access their records,
contact the licensing authority, and submit consumer complaints.

Accessing Your Health Care Records

Individuals who have received psychological or mental health services from H. Denise Wooten,
PsyD may request access to their clinical records in accordance with applicable state and
federal privacy laws.

Requests may be submitted through any of the methods listed below:

To help ensure timely processing, requests should include:

● Client’s legal name and date of birth
● A current email address or phone number
● A clear description of the records being requested (including relevant dates, if known)
● Preferred format for receiving records (secure electronic delivery, mail, or in-person
pickup)
● Client signature and date

Requests submitted by a legal representative must include documentation demonstrating authority to access the records. Verification of identity may be required to protect confidentiality.

H. Denise Wooten, PsyD is licensed and regulated by the Texas Behavioral Health Executive
Council (BHEC)

Submitting a Consumer Complaint

Consumers who wish to report a concern regarding professional services may submit a
complaint through the Texas Office of the Attorney General – Consumer Protection
Division.
